Several: Common is pulmonary atelectasis from poor cough effort and pain control early after surgery. But urinary, wound and other causes will be checked. Later still needs checked by surgeon or team.

Fever: How long ago was surgery? A few days ago? If so, he needs to see his surgeon immediately and get some bloodwork done and be examined to rule out a leak or abscess.
