Yup: All the food and bacteria in that lesion decay and release malodorous fumes. Have it treated at a local dentist.

Yes: A large cavity can trap food particles then bacteria and plaque form . The waste products formed are acid in nature and can give you bad breath. Fix it!

Yes but,: If you have bad breath there are also other possible causes. Just because you know or think that you have a large cavity, doesn't mean that you also don't have gum problems, etc. Regardless, it seems like it's time to see your dentist.
