Surgery: If you have an extra $100k and years of your life you don't mind loosing to recovery, then it might be feasible. Surgery to make you taller is not covered under insurance and is significantly expensive. With that said, there's no way to tell how much your smoking habit decreased your potential for growth.

Not sure.: The natural history of nicotine dependence in teens study (funded by canadian cancer society) published in journal annals of epidemiology (17 march 2008) showed that on average, teenage boys who smoked were 2.54 cm shorter than their non-smoking counterparts. I have not reviewed study – so i can’t comment on quality of research. Most males stop growing taller by around age 18. I wouldn't look to.
