Why gout?: Gout does not commonly affect the shoulder. While nothing is impossible, i would say she needs an evaluation to see what is the cause of her pain. Once it is identified she can start with some anti-inflammatories and physical therapy to target the cause of her pain. Depending on the diagnosis she may also benefit from a cortisone shot directed at the location of her pain generator.

If it is gout....: If it is gout, that is if gout (uric acid) crystals are found in fluid removed from the shoulder, low does colchicine, a non-steroidal anti-inflammatory drug or acth intramuscular injection should help - dependent upon her other medical problems and medications.
