4yr old f, rt leg pain 5 month.no specfic time, duration, no trama, no infect, fever.tight hip at birth, not treated.what could be the cause?athritis?joint?

Reply: Is the pain associated with any joint swelling, warmth, limping etc? Was she born breech? What was the exact diagnosis at birth, if it was congenital hip dysplasia, then that was supposed to be treated. Now can start from basic imaging of hip and knee joints, some blood tests and full exam of the leg and her walk to decide further.

Leg pain: Leg pain in a child deserves a comprehensive workup. Start with your pediatrician who can order routine blood work including a sed rate. He can also order x rays of the hips and legs. A bone scan may also be in order. Causes can be anything from flat feet to legg perthes disease or diskitis depending on the results of your blood work and xray/bone scan results.

See ortho or rheum: Most young children with arthritis do not complain of pain, but some do. See a pediatric rheumatologist or pediatric orthopedic surgeon for an evaluation. A thorough history, physical exam, x-ray films, and basic blood work can help rule out many causes. Sometimes hip ultrasound is helpful. In other cases, an MRI with contrast can detect arthritis not apparent on physical exam or plain x-ray films.

See specialist: The child should be seen by a pediatric orthopaedic doctor. A diagnosis cannot be made without a good history, examination, and imaging.
