7 days: Typically, i tell patients to wait 7 days after starting a new pack of birth control pills before having unprotected sex. In your case, you had the Mirena (levonorgestrel) and you didn't skip a day, and you're 46 years old, so chances are super slim of pregnancy. However, if you want to be extra cautious, take the progestin pill for 7 days and then have unprotected intercourse. If you had sex... Don't worry!
