See a dentist: A professional can assess whether or not your retainer is worn to the point of replacement and if it is functioning as designed.

Depends: Depends on the design and how often you wear them. If it is clear aligner style, it may crack or stretch and therefore not work well, often replace once per year. If hard plastic with wires (like the photo), they may last longer. If you see your dentist twice per year, bring your retainers for their evaluation.
