Anterior: Anterior is the description of the anatomical position which means in the front, as opposed to posterior, meaning behind. So your imaging description, typically MRI will often use these terms to explain the anotomy visulaized.

Between portals: Scar tissue can form deep to the patellar tendon, within the fat pad that lies between the two anterior portals in the front of your knee. Though nonoperative treatment and physical therapy is preferred, repeat surgery to release and debride the irregular tissue is ocassionally indicated to regain motion and decrease pain.

Scars or keloids on : Incision site may happen after a procedure and unfortunately some people are more prone than others. It's best to leave them alone but u can have them remove for cosmetic reasons. There's no guarantee though that it won't come back. Take care!

Post op changes: Assume MRI suggested this diagnosis. Typical after this sort of surgery. Suggest good trial with pt as further surgery will create more scar tissue. See your ortho surgeon again if you haven't been there lately.
