Variable: COPD has multiple stages. People can live decades and normal lifespans with copd. Depends on the rate of decline in lung function. Stages used to be numbered 0 to 5. Stage 5 very severe generally life span if not getting sick/pneumonia/respiratory failure 5-10 years still.

Hard to answer: Of c course frequent need for a rescue inhaler, hospitalizations, need for oxygen, etc are indications. Late manifestations are also seen in progressively diminishing spirometry studies. There are other symptoms that can best be discussed with your pulmonologist.

Imbalance: We have chemicals called proteases which can break down tissue and anti-proteases which turn them off. People susceptible to COPD have an excess of proteolytic enzymes which break down the alveolar wall over time.

A few: Triggers in COPD do differ often from those in asthma. Respiratory infections, inhalation of particulates (ie smoke, powder, etc) are common. Smog and industrial exhausts can also be a trigger.
