Maybe: You seem to be getting worse by the fact that ur meds don't work. The pain in the upper rib cage may represent a different and possibly a more serious problem. Get in to see ur pulmonologist.

Possibly: At this point if your medicines aren't helping you breathe better it's time to start thinking about using medicines which are for relief of the symptoms of copd. These include benzodiazepines ( valium like medications) or even opiates (like oxycodone) since these are very effective at relieving shortness of breath. Talk to your health care provider about your symptoms and goals.
