? appendix testis: If pain is confined to tiny tender swelling on upper end of scrotum. Epididymitis or epididymal cyst could all produce these symptoms. Suggest you see a urologist, who may well advise an ultrasound exam. Epididymitis means antibiotics & rest, epidymal cyst -> observation or excision depending upon severity of symptoms, similar for rorsion of appendix testis.
