Puncture: Try soaking in warm water with epsom salt, then use tweezers to remove the object. If it hurts, then it is usually a simple office procedure to remove the foreign body.

Depends: You need to clean the wound and if you canseethe punture site apply an antiseptic to it. I would make sure that your tetanus vaccination is up to date.
