2nd toe: With big toe leaning on second toe it sounds like you have a bunion causing the problem. Try placing some padding between the toes. Due to pain at base of toe you may have capsulitis or joint problem. See a podiatrist for diagnosis and treatment. Good luck to you.

Metatarsalgia: You may have joint capsule pain or plantar plate ligament sprain. It is common with elongated 2nd ray (toe and metatarsal bone) and a tight heel cord. Overload by walking or repetitive trauma can lead to swelling and pain. Also, if you have a bunion, the great to leaning into the 2nd toe can cause disruption of the joint and deformity.
