Clot: It is important to determine if the clot is in the coronary artery causing ischemia/infarction or another artery. He needs to be hospitalized immediately if we are talking about the coronary artery. The treatment i to either place a stent or surgically bypass it. Less common these days is dissolving the clot with thrombolytics (clot busters). Other arteries are treated with surgery or medicatio.

Tight: I assume the heart has beeb thouroughly checked. Other possibilities w ould be asthma, esophageal spasms (from acid) or coronary artery spasms. It could also be muscular.
