Normal: A fetus with a heartbeat is not visible on ultrasound until 6 weeks. Do not worry at this point.

Corpus luteum: You typically wouldn't see anything in the uterus at 4 weeks yet assuming your dates are correct. In addition you should have a cyst on the ovary called a corpus luteum (latin for yellow body), which produces the Progesterone needed to maintain your pregnancy until your placenta takes over production around 12 weeks. Ask your provider if thats what the cyst likely is and get a follow up scan.
