Scar : At the skin, i'd recommend asking about use of Mederma or other silicone based scar products. It's it's scar internally, possible physical therapy with ultrasound to breakup the scar tissue may help.

Multiple reasons: Can ask about skin products, cortisone, physical therapy and further surgery. Ask if a nerve is involved in the scar tissue. Sometimes topical medications help. There could also be a change in the way your foot works due to the fusion. This is best evaluated by the surgeon.

WHAT IS IT?: Is it pain from scarring on the surgical incision itself, or deeper, underneath the skin that feels like a solid ball? Also to consider; maybe the fusion didn't take, and that's causing the pain. Talk to your surgeon to make sure he/she is confident the fusion is solid.
