Couple possiblities: You are a younger person with knee replacements. One reason is you simply wore them out. With a cruciate retaining knee and now with tears of the PCL you are putting more strain across the implants as the ligament has failed. Could be that surgery was done poorly. Could be infection although less likely given bilateral failure. First two most probable.

Difficult to know: Loosening is often an enigma. Infection can cause or trauma such as a fall. A certain percentage gets it and we do not know why. There may be factors we don't understand such as biological reactions to the implant that may happen in only certain people with a certain genetic constitution. Apart from revision with stems not much anyone can do.
