Being male, my left nipple has a hard swell beneath it that is no larger than the halo and hurts when pinched. could this be a tumor?

It may be normal : It may be normal at age 18 in a male. But i will recommend that you have it checked by your doctor to make sure it is not a tumor, which is very unlikely but for your satisfaction.
