No: There is a maximum of 1 week difference between measurements and dates in the first trimester. Perhaps your dating was incorrect to start. Otherwise there is an issue with fetal growth.

Unlikely: A 13 week ultrasound is very accurate if done by a well-trained doc or tech. Growth abnormalities usually don't surface until later in the pregnancy & the difference between a 9 and 13 weeks fetus is easy to see. If you notice the 4 week discrepancy, it is much more likely that an ovulatory cycle was missed.
