Splenic Rupture: 0.1% to 0.2% of patients with mononucleosis experience a ruptured spleen.

Depends: You are more likely to experience this type of complication if you are involved in a fall or other trauma while the spleen is enlarged. Most recommend avoidance of such things as contact sports, bicycle or horseback riding and similar endeavors.
