External hemorrhoid: I suspect this is an external hemorrhoid. These vascular extensions of the local veins often develope in people who are constipated or chronically strain too hard while defecating. Chronic straining places high pressure on the rectal veins, causing these vascular 'balloons'. Narcotic pain medications can cause constipation, causing this problem. Less likely causes would be an infectious agent.
