So i have few patients which i can't determine the cause for low ferritin. i have ruled out celiac, bypass hx, h. pylori, vaginal bleeding, gi causes.

May not find cause: In some cases we do not find the loss of iron. Treatment is still important. I have seen a few cases where the source of the loss is discovered later in the course.

Good question: I'm glad you're being thorough. Angiodysplasia of the colon bleeds intermittently and can drain iron without being picked up on routine screen; even colonoscopy may miss it. A patient may not tell you they are a vegetarian / vegan. And i'll bet there are a few causes unknown to science. Best wishes.
