Depends: If only dry eyes and dry mouth, would recommend only treatment of symptoms. If there is extraglandular manifestations (such as inflammatory arthritis, neuropathy, other internal organ involvement), then immuosuppressive treatments are used.

There are several.: I am following over 100 pts with primary Sjogren's. If Plaguenyl cannot be used, oral gold (Ridaura) is an excellent option. A new drug, not yet indicated for Sjogren's, but in FDA approved trials is Rituxan (rituximab). Anecdotal reports suggest promising things. No other drugs are of agreed upon use. I believe "pulsing" with dexamethasone is of use, but no trials exist.
