Lower back pain trea: Lower back pain has many etiologies and treatments. After a thorough examination and diagnosis, the most important treatment is to include specific and focused physical therapy - stretching and exercise. Next, interventional therapies, which are determined based on the cause of the lower back pain, as indicated. Lastly, is medications for breakthrough pain. All can be used together.

LONG TERM OPIATES: Unfortunately long term opiate use does not work. Every so often one has to increase the dose just to get the same effect. If the patient stops suddenly then the pain goes way up because of withdrawals. If the pain meds get too high a reverse effect occurs where the opiates actually cause more pain. Suggest non opiate treatments, facet injections, pt, ect. Get help before stopping opiate.

Consult a pain MD: There are multiple causes for low back pain and the treatment options vary with the cause. Consider consulting a pain management physician who performs interventional techniques. Some causes are very effectively treated with procedures and can reduce or eliminate the need for narcotics.
