Multiple issues: First thins you should do is lower your cardiovascular disease risk factrods. See your dr and get on metformin, statins, Aspirin and/or blood thinner, and possibly beta blocker & an ace inhibitor. Discuss optimal weight & diet with your doctor. May need MRI brain to evaluate the severe headaches.
