Is there always a underlying condition like lupus when you have + ana's and decreased gas exchange in lungs or could it be from smoking.

Problems with gas exchange suggest restrictive rather than obstructive lung disease. Restrictive disease is not associated with smoking but is not uncommonly a complication of lupus.
