Is there always a underlying condition like lupus when you have + ana's and decreased gas exchange in lungs or could it be from smoking.

See details: Problems with gas exchange suggest restrictive rather than obstructive lung disease. Restrictive disease is not associated with smoking but is not uncommonly a complication of lupus.
