Cervicogenic? APNEA: I would be suspicious of a sleep disordered breathing problem. Headaches at night are usually associated with sleep problems where your oxygen concentration can fall during your sleep. High blood pressure can also be the result of sleep apnea. History of high bp, pain meds, abdominal pain are red flags with the headaches. The neck might be from forward head posture to maintain airway. See md.
