Various: Acute severe pain in the testis might be testis torsion & an emergency. Severe pain of fairly rapid onset & relieved by gently lifting scrotum on that side could be epididymitis requiring antibiotics. Dull ache could be due to varicocele or hydrocele. Pain can be related to sexual stimulation without ejaculation. Suggest see a urologist if there is any doubt.
