Many things: Testicular or scrotal pain can be caused by a variety of things. If the testicles are palpably tender, epididymitis is likely the cause. If there is no palpable irregularity or tenderness to the testicles or scrotum then referred pain from prostatitis would be a likely cause. Other things such as inguinal hernia or lumbar radiculopathy are also possible.
