Complaint?: How long has it been? What makes it worse or better. What have you tried to help? What medicines do you take? What have you been treated for? Anything run in the family? How's life outside of this problem? Are you hurting anywhere? Have you been gaining or losing weight? Sleeping, eating ok? Have you ever been hospitalized? What kind of work do you do? Do you smoke or drink? Married, have children.
