Many things: It could be many things. Meniscus tear, strain or tear of the cruciate ligaments, tear of patellar tendon, collateral ligament tears, bone contusions, osgood schlatters disease and many others. If you have not responded, it may be necessary to have an MRI of the knee done to help make a diagnosis. Depending on your age, it is is arthritis, you may need to see an orthopedist.
