Not sure: You mention an "md" who is "reluctant to medicate" -- but you don't specify his/her specialty. One reason a doctor might be reluctant to medicate could be substance abuse -- where the patient isn't willing to stop using. Doctors may recommend treatment, but patients can refuse. If not seeing a psychiatrist, please do so asap. Psychiatrists have specialized training & may be better able to help.

No: It depends on the severity. Symptoms of depression include : a loss of interest in activities you used to enjoy, difficulty concentrating, change in sleep and/or appetite, fatigue, feelings of hopelessness, sadness, poor motivation, and irritabiliy. These symptoms can adversely affect your ability to function at home and/or at work. In severe cases suicidal ideation.

If you have thought of a way to kill yourself and believe you could act on it or your thoughts are scaring you, please get to the nearest emergency room now. Call 911 for transport if you need to. If they are vague thoughts of suicide, please see a psychologist or a psychiatrist promptly for evaluation. They will be able to determine if you have depression.
