It depends on how fast the rhythm is and what your symptoms are as well as how long it lasts and what your blood pressure is doing during the abnormal rhythm whether treatment is needed. The initial treatment would be medications to keep your heart rate slower, as long as your blood pressure is able to tolerate the medicine. Some medicines for heart rate also affect blood pressure and others don’t

Consult a cardiology electrophysiologist. Accelerated idioventricular rhythm, when benign, is caused by some interplay between increased vagus nerve tone and decreased sympathetic tone. Since you're experiencing frequent, very symptomatic palpitations, I believe some treatment may be indicated. A lot depends upon how physical activity interrelates with heart rhythm & symptoms.
