I assume the culture showed no infection. If this was a well performed clean catch specimen, protein in the urine and abnormally high numbers of red blood cells in the sediment raises concern for glomerular kidney pathology such as a nephritis but stones,tumor are other possibilities. More studies like 24 hour protein,U/S,immunofixation are needed and a nephrologist is best to direct the workup
