Can gonorrhea spread through semen even if it is rectal gonorrhea?

No. Gonorrhea does not travel through the body to infect places other than the originally exposed site. And there is no internal connection between the rectum and the genital tract, even though they are close to each other. This is also why each exposed site must be tested: negative urine/urethral sample says nothing about infection in the rectum or throat.
