The brain results may be nonspecifically related to aging, microvascular angiopathy, migraine, and smoking, but could be further signs of the pathology noted initially in 2013. This needs to be sorted out and handled. This is NOT a surgical issue, and rather you would benefit from an experienced and seasoned neurologist.
