It depends-is the person anticoagulated? Amiodarone can potentially put the person into normal sinus rhythm so the person needs to be on blood thinners. THe other point to be made is that if the person is not having any symptoms and especially if they stay in afib, I would not use amiodarone-too many potential side effects to treat something that is not causing any issues. With this age and very advanced comorbidities I would only emphasize rate control with beta blocker or, if needed, low dose digoxin checking digoxin levels carefully & regularly keeping it under 1.0. The goal is to keep ventricular rate in 70's. In addition lower dose anticoagulant is needed. Collaboration with gastroenterologist & nephrologist would be important especially if dialysis is used.
