The current ultrasound may show a second polyp as described, but uterine polyps are generally benign, and unless bleeding occurs, do not usually interfere with fertility. An ultrasound may not always show all uterine abnormalities, and the hysteroscopy may have effectively removed one and the "new" polyp may have been missed.
