New HSV outbreaks, either from new infection or from self-infection (medical term auto-inoculation) is extremely rare. So most likely the new problem isn't herpes. However, it should be tested with HSV PCR. If herpes is confirmed, or just to evaluate the reason you have particularly severe cold sores, see an infectious diseases specialist. You may also need testing for acyclovir resistant HSV.
