If new, not normal: Pain in the middle of the back is a little unusual for a kidney problem. The kidneys are more on either side. A kidney infection more usually presents with burning or pain with urination, and sometimes going more often.Fevers can be the sign of a more serious infection.Pain in the middle of the back could involve a bone.Severe pain may suggest a compression fracture or something else to check out.

Could br kidney: Usually pain related to the kidneys is on the sides and travels to the belly if it is caused by a stone. If you have fever or burning on urinating you may also have a urine infection and then you will need cultures and antibiotics. If pain is related to movement it is most likely muscular or due to bones.
