I got two very fine sutures taken out of my upper chest after getting a punch excision about a week ago. it now looks worse than the original wound. i was told it's supposed to be a line but instead it's all puffy and there's indentations sutures?

The sutures are tight. Apply aquaphor ointment daily. If there's no inflammation or wound infection,I predict the wound will heal nicely. The surgeon or his assistant will advise you about your wound when they remove the sutures.

You should have to looked at by the doctor who did the biopsy. Is the site red? Any pus coming out of it? Are there sutures still left in? Is it painful? In the meantime, you should clean it and put some antibiotic ointment-like bacitracin or triple antibiotic ointment-on the site, just in case the area where the sutures were removed is getting infected. But, call your doctor to evaluate it.
