I am about to start methotrexate for rheumatoid arthritis and have had genital herpes in the past. how common will outbreaks occur on it and to what severity?

The usual doses of methotrexate for RA are quite low and do not typically cause serious immunosuppression. There are a few reports of herpes complications, but it appears to be a rare problem. There are no data on frequency of recurrent outbreaks of either oral or genital herpes associated with methotrexate. Be prepared for possible problems, but probably nothing will happen. Good luck.
