At the bottom of my recent lab tests, it says chronic kidney disease: less than 60 ml/min 1.73 square meters end stage renal disease: less than 15 ml/min 1.73 square meters what does this mean?? is there a reason to be concerned?

This is saying the parameters for the diagnosis of chronic kidney disease and end stage renal disease. You don’t indicate what your result was. So, if your result is less than 60, that indicates chronic renal disease and less than 15 indicates end stage renal disease. You need to look at your result and see where it falls. The doctor who ordered the test can interpret it for you. Good luck!
