You ask a very good and difficult question. the description sounds like the complete leg is internally rotated, and it could be from the hip that being the case an orthopedic surgeon who deals with children, or a children’s hospital, might be the place to bring such a case. However, sometimes a mild case a podiatrist can also have input.

The hips and extremities should be evaluated by Pediatric Orthopedics. They may refer you to Pediatric PT. Musculoskeletal and joint inflammation or infection should also be considered. Watch for signs of fever and pain
