Esr 41 , + for maxilla bone biopsy bacterias , on iv picc line antibiotics and white count trending up and esr elevated again , sinus feels irritated back of nose cheek and now eye is swollen, pre molar feels off -sinus or tooth?

The elevated ESR (erythrocyte sedimentation rate) and white blood cell count, along with the presence of bacteria in a maxilla bone biopsy, suggest an active infection. The irritation in the sinuses and back of the nose and cheek, as well as the swollen eye, are also indicative of sinus inflammation. It could also be related to a dental issue spreading from the premolar.
