If you are experiencing an infection of the nail and skin usually means the surrounding soft tissue is being inflamed and irritated causing a break in the skin thus an infection. See a podiatrist and have it cared for properly. Usually part of the nail needs to be removed and the cause needs to be determined

Yes, by definition, a toenail paronychia is pus under the skin, even though it may not be visible. The swelling is usually caused by pus underneath the skin. Surgical drainage is usually needed to treat this.
