I have a toenail paronychia. one week ago began topical mupirocin without success. two days ago began cephalexin 500 mg 3 times per day. no improvement yet. no visible abscess to drain. no other adverse symptoms (no fever, etc.) what to do next?

It might take a few more days. However, paronychia inflammation isn't necessarily due only to infection. You might require minor surgery to drain it and remove the corner of your nail. See your doctor if it isn't better after another week.
