Tested pos for rectal gonorrhea in march- got antibiotic inj and signs got a bit better but suspect ive had it since jan (partner slept with pos+ person around january) still having gi issues- is it possible my infection spread and i need more tx?

Dual therapy using ceftriaxone intramuscular injection with azithromycin (1 g, orally), administered together on the same day, is current standard treatment for rectal gonorrhea. In case you did not receive the dual therapy, I would recommend to get treated again.

First, the standard antibiotic injection for rectal gonorrhea (ceftriaxone 500 mg) is effective 99% of the time. You were cured. Second, rectal gonorrhea does not spread elsewhere in the GI tract. If you're have rectal/anal symptoms (discharge, pain, etc) return to your doctor for possible testing for other STDs or (just to be sure) persistent gonorrhea.
