Your medication needs to be managed by a Psychiatrist. If you are not confident in your Dr. please see another for a second opinion. Medication alone may not be enough to help. Please see a Clinical Psychologist with expertise in multiple types of psychotherapy to help. Also it may be time for a complete physical exam to know if any of your symptoms are due to a physical illness
