Possibly, you might have Radiologically Isolated Syndrome, and a new lesion in the spinal cord might be a clue to clinically definite MS. But shoulder and neck pain may be due to problems in the joints or brachial plexus, which would not be due to MS. Believe in your case that a second opinion by a neurologist who focuses on immunological disorders might be a valuable next step.
